You have a right to your records, to contact the council, and to file a complaint.
Section 181.105 of the Health and Safety code require that we make it easy for you to find information on how to request your healthcare records, contact the Texas Behavioral Health Executive Council, and file a consumer complaint with the Office of Attorney General’s Consumer Protection webpage. Email me a formal, written request for your records at kaleigh@moonflowercrb.com. Include what kind of records you need (progress notes, psychotherapy notes, or dates of sessions), what time frame you need them in, and who the recipient is. If you are not the recipient, I will send you a Release of Information form to release your records.
